Regulatory Overview

What are cardiovascular devices?

Cardiovascular devices are the medical devices used to diagnose, monitor, and treat diseases of the heart and blood vessels. The category is unusually broad. At one end are high-risk implantable therapies: coronary and peripheral stents, pacemakers, implantable cardioverter-defibrillators, prosthetic heart valves, and structural-heart devices such as transcatheter aortic valves and left-atrial appendage closure devices. In the middle sit the interventional tools that deliver them, including catheters, guidewires, balloons, and closure devices. At the other end are diagnostic and monitoring products: ECG systems, Holter and event monitors, mobile cardiac telemetry, cardiac output monitors, and the wearable rhythm patches now common in remote monitoring. Mechanical circulatory support, such as ventricular assist devices and percutaneous heart pumps, sits at the highest-risk edge of all.

Because the same category runs from a wearable monitor to an implanted valve in contact with circulating blood, cardiovascular devices span nearly the entire regulatory risk scale, and every jurisdiction draws the lines differently. A cardiac monitor may be a Class II product cleared in months, while the stent or valve beside it is a Class III device that needs a multi-year clinical program. That is why a structured classification analysis across your target markets, not a single global assumption, is the first deliverable of any serious cardiovascular program. Every cost below has two parts: the government fee, and our own flat annual service fee per market, from US$1,000 per year in the US (device listing and US Agent representation) and from US$2,000 per year in most other markets.

FDA cardiovascular device regulation (United States)

FDA regulates cardiovascular devices through its risk-based framework, and the category sits at the demanding end of it. CDRH's dedicated Office of Cardiovascular Devices reviews cardiovascular products, convening its Circulatory System Devices Panel for novel high-risk approvals, and life-sustaining implants carry the agency's most rigorous premarket and post-market requirements.

  • Classification. Implantable and life-sustaining devices, including coronary stents, pacemakers, ICDs, prosthetic heart valves, and ventricular assist devices, are Class III and require a Premarket Approval (PMA). Diagnostic and monitoring devices such as ECG systems, Holter monitors, and mobile cardiac telemetry are mostly Class II with a 510(k). Genuinely novel low-to-moderate-risk devices without a predicate can take the De Novo route; the classification memo settles this first. Drug-eluting stents are drug-device combination products, reviewed under a PMA with CDRH as lead center and CDER consultation.
  • Timeline. A Class II 510(k) typically runs 3 to 9 months including preparation. A Class III PMA is a multi-year program, often 1 to 3 years, because it usually rests on a prospective clinical trial and may go to an advisory panel. A Pre-Submission meeting adds a few weeks up front and routinely saves review cycles later, especially for novel structural-heart devices.
  • Cost. Government fees: US$26,067 for a standard 510(k) review (US$6,517 for qualified small businesses), plus US$11,423 per year in establishment registration; a PMA carries a substantially higher review fee. Cardiac assist devices and mechanical circulatory support

Mechanical circulatory support, which includes ventricular assist devices (VADs), total artificial hearts, and percutaneous heart pumps, represents the highest-risk end of the cardiovascular field and one of its most-searched. These are Class III devices in the United States, approved through PMA and, for smaller patient populations, through the Humanitarian Device Exemption. Because many are genuinely novel, Breakthrough Device designation is common and can bring earlier, more frequent FDA interaction without lowering the evidence bar. The clinical, manufacturing, and post-market commitments are the most demanding of any device type, and the same rigor carries into every other market, so the anchor-market clinical dossier does most of the global work.

EU MDR cardiovascular devices: Class III classification and CE marking

Under the EU Medical Device Regulation (2017/745), cardiovascular devices that contact the heart or the central circulatory system are pushed to the top of the risk scale, and a notified body sits in the middle of every high-risk timeline.

  • Classification. MDR Rule 8 places implantable devices in Class IIb by default, but devices intended to be in direct contact with the heart or the central circulatory system, such as stents and heart valves, are Class III, and active implantables such as pacemakers and ICDs are Class III as well. Non-invasive diagnostic and monitoring devices sit lower, typically Class IIa. Class III implants are also subject to the MDR Article 54 clinical evaluation consultation procedure, under which an independent EU expert panel reviews the notified body's clinical evaluation assessment. Drug-eluting stents carry an extra step: as devices incorporating an ancillary medicinal substance (Annex VIII Rule 14), the notified body must also obtain a scientific opinion from EMA or a national medicines authority on the drug component, a consultation that must be planned into the certification timeline.
  • Timeline. Plan 12 to 24 months for a first Class III certification with a notified body, driven by notified body capacity and the maturity of your technical documentation and clinical evaluation. The MDR clinical evidence requirements and, for implants, the Summary of Safety and Clinical Performance are usually the critical path.
  • Cost. There is no central government fee; the money goes to the notified body, typically EUR 30,000 to EUR 70,000 for a first Class IIa cycle and higher for Class III, plus annual surveillance and the cost of MDR-grade clinical and technical documentation. Cardiovascular device registration in Brazil and Latin America (ANVISA RDC 751)

Brazil anchors any Latin American strategy, and ANVISA classifies cardiovascular devices under its risk framework (RDC 751/2022) on the same four-class model used worldwide. Mexico's COFEPRIS is the region's second gate. Both require a local representative, and we act as Brazil Registration Holder without taking control of your registration.

  • Classification. Under RDC 751, lower-risk cardiovascular products such as ECG monitors qualify for the streamlined notificacao route, while implantable and life-sustaining devices, including stents, valves, and pacemakers, are Class III or IV and require full registro with deep technical and clinical evidence. High-risk implants generally also need international B-GMP certification of the manufacturing site.
  • Timeline. Notification is typically a matter of weeks; Class III-IV registro plans for 6 to 12 months, longer when a B-GMP inspection is on the critical path. In Mexico, COFEPRIS runs 6 to 12 months on the standard route, faster where reliance on an FDA or CE approval applies.
  • Cost. ANVISA government fees: R$1,406 to notify a Class I-II product; Class III-IV family registration runs R$8,510 to R$19,856, plus a one-time international B-GMP certification of R$72,805 where required. COFEPRIS charges MX$16,499 to MX$30,798 per product by class. Our registration service starts at US$2,000 per year, and US$3,000 for high-risk classes, in both markets.

Cardiovascular device registration in Asia-Pacific: Singapore first, then ASEAN

Most overseas cardiovascular teams enter Asia-Pacific through Singapore: HSA works in English, follows the global risk model, and rewards a strong FDA or CE dossier with a faster abridged review. A Singapore approval then anchors expansion across ASEAN, including Malaysia, Thailand, Indonesia, Vietnam, and the Philippines, where reliance-friendly frameworks make each additional market incremental rather than a new program. Japan and Korea are the region's large, mature prizes, each with its own system, language, and clinical expectations. China is the largest market but the hardest entry: high-risk cardiovascular implants generally require local clinical data and type testing and the longest timelines, so treat it as its own program when the business case justifies it, not as a default stop.

  • Classification. Singapore's HSA uses risk classes A through D, and implantable cardiovascular devices land in Class C or D. ASEAN members track the same model. Japan classifies against JMDN codes, Korea's MFDS uses Classes I-IV, and China's NMPA places most cardiovascular implants in Class III, its highest tier.
  • Timeline. HSA abridged evaluation with a reference approval typically closes in 4 to 10 months for higher-risk devices; ASEAN registrations run 3 to 9 months per market on the same dossier. Japan plans for 12 to 18 months via PMDA, Korea 6 to 12 months including KGMP, and China 2 to 3 years including local clinical work and type testing.
  • Cost. Government fees are modest relative to the clinical spend: Singapore charges an SGD 560 application plus SGD 2,010 to SGD 6,250 evaluation by class; Malaysia MYR 500 to MYR 3,000; Thailand THB 3,100 to THB 21,000, with an added expert-review fee where required. Japan's PMDA review fees run from roughly JPY 1 million for me-too devices to about JPY 13-18 million for new Class III-IV device reviews; China's NMPA fees for imported Class II-III run roughly RMB 210,000 to RMB 310,000 before type-testing and local clinical costs. Cardiovascular device registration in Saudi Arabia and MENA (SFDA)

The Gulf is among the fastest-growing regions for cardiovascular device launches, and its regulators are built around reliance: a strong FDA or CE approval does much of the work when the dossier is assembled correctly. SFDA regulates cardiovascular devices on the global risk model and requires a local Authorized Representative.

  • Classification. SFDA classifies devices into risk classes A through D, mirroring your reference-market class in most cases, so a Class III implant in the US or EU is treated as high-risk in the Gulf. The UAE's Emirates Drug Establishment (EDE), which took over medical device registration from MOHAP in 2025, and other MENA authorities lean on the reference approval's classification.
  • Timeline. With a reference approval in hand, SFDA medical device marketing authorization (MDMA) typically closes in 5 to 10 months for high-risk devices; UAE registration runs a similar range. Without a reference approval, expect materially longer.
  • Cost. Government fees are modest relative to the clinical spend: SFDA's marketing authorization fee runs about SAR 15,000-23,000 (roughly US$4,000-6,000) by class, and other Gulf authorities charge less, so the real spend is dossier assembly, Arabic labeling where required, and local representation. Saudi Arabia and the wider Gulf are not in our public calculator; we quote these markets flat per market on the same transparent model.

Cardiac monitoring and wearable cardiovascular devices

Cardiovascular monitoring devices are the highest-volume search theme in this category after the implants themselves, and it is where the regulatory line is easiest to miss. ECG systems, Holter and event monitors, mobile cardiac telemetry, cardiac output monitors, and wearable rhythm patches are mostly Class II in the United States and clear through a 510(k), with performance data and, increasingly, cybersecurity evidence for connected products. Consumer wearables sit on a boundary: a watch that reports heart rate for general fitness is usually outside device regulation, but the moment a feature claims to detect atrial fibrillation or record a medical-grade ECG, that specific function becomes a regulated device, typically Class II and often via De Novo the first time a function is introduced. In the EU these monitoring devices are generally Class IIa, and any algorithm-driven analysis pulls software and cybersecurity obligations into the file. We help teams draw the wellness-versus-device line before launch, market by market, so a claim does not quietly turn a consumer product into an unregistered medical device.

Evidence, quality system, and lifecycle

Cardiovascular devices carry the heaviest clinical evidence burden of any device category, and it shapes every program. High-risk implants generally require prospective clinical investigations run to ISO 14155, with follow-up measured in years for endpoints such as thrombosis, restenosis, valve durability, and lead integrity; FDA PMA and EU MDR both expect this evidence, and MDR added a mandatory clinical evaluation and, for implants, a Summary of Safety and Clinical Performance. We plan the clinical strategy once for reuse across FDA, EU MDR, and Asia-Pacific submissions, set up ISO 13485 and FDA QMSR quality systems, and, because more cardiovascular medical devices now carry wireless telemetry and companion apps, maintain cybersecurity documentation that satisfies FDA, EU MDR, and IEC 81001-5-1. After launch, vigilance reporting, field-safety corrective actions, and the post-market clinical follow-up that high-risk implants require keep every registration current. In the US, many high-risk cardiovascular implants — pacemakers and their electrodes, ICDs, mechanical replacement heart valves, and ventricular assist devices — also carry FDA device tracking obligations under 21 CFR Part 821.
