EU MDR PSUR: Periodic Safety Update Report
Last reviewed:
Prepare a device-specific PSUR that connects postmarket evidence, benefit-risk conclusions and corrective actions under MDR Article 86.
What is an EU MDR PSUR?
A Periodic Safety Update Report (PSUR) summarizes the results and conclusions of a manufacturer’s post-market surveillance analysis and explains preventive or corrective actions. Under MDR Article 86, it is required for Class IIa, IIb and III devices. It is a lifecycle regulatory record, not a substitute for the PMS plan, complaint investigation or urgent vigilance reporting.
The report should explain whether real-world experience still supports the device’s benefit-risk profile and intended purpose. A collection of complaint counts without denominators, analysis or conclusions is insufficient. MDCG 2022-21 provides the Commission-endorsed practical guidance for MDR PSUR preparation.
PSUR versus PMS Report: which frequency applies?
| Regulation and class | Periodic output | Minimum update framework |
|---|---|---|
| MDR Class I | PMS Report under Article 85 | Update when necessary |
| MDR Class IIa | PSUR | When necessary and at least every two years |
| MDR Class IIb and III | PSUR | At least annually |
| IVDR Class A and B | PMS Report under Article 80 | Update when necessary |
| IVDR Class C and D | PSUR under Article 81 | At least annually |
Apply the IVDR separately; its Class B rule is not the MDR Class IIa rule. Legacy-device applicability and transition conditions also need assessment. Urgent signals, serious incidents and field actions cannot wait for the next periodic cycle.
A practical PSUR evidence map
| Report component | Evidence to reconcile | Question the analysis should answer |
|---|---|---|
| Device scope and reporting period | Models, identifiers, versions and grouped-device rationale | Which devices and time interval do these conclusions cover? |
| Exposure and use | Sales, estimated user population and use frequency where practicable | What denominator makes the event rates interpretable? |
| Safety/performance signals | Complaints, serious incidents, trends, literature and other PMS sources | Are rates, severity or patterns changing? |
| PMCF findings | Completed activities, limitations and remaining gaps | Do clinical data support the claims and residual risks? |
| Benefit-risk conclusions | Risk-management file, clinical evaluation and state of the art | Is the current benefit-risk determination still acceptable? |
| Actions | CAPA, labeling changes, field actions and future work | What changed, who owns it, and how will effectiveness be checked? |
Use favorable and unfavorable information and explain data limitations, low exposure, missing follow-up and uncertainty. Keep the method consistent across periods or explain why a changed method affects comparison. Grouping devices requires a defensible rationale; a commercial product family is not automatically a suitable reporting group.
Review, availability and EUDAMED
Define data owners, cut-off dates, analysis methods, report author, reviewer and approval authority. Verify the relationship between the reporting interval, device history and the required cadence. Map each conclusion to evidence and each action to a controlled record.
MDR submission/review arrangements differ by device category, including Class III and implantable devices. Confirm the applicable route with the Notified Body and current transitional reporting arrangements. The EUDAMED vigilance/PMS module must not be described as universally mandatory simply because four other modules are in mandatory use.
Connect the PSUR to clinical and quality records
A new safety signal can require changes to risk management, the clinical evaluation, PMCF plan, labeling or design. Record those decisions and their implementation. A PSUR update that leaves a contradictory CER or risk file unchanged is not a complete lifecycle response.
Use the EU MDR service pathway for the wider conformity program and PMS guide for the system generating the data. Pure Global can help define scope, reconcile inputs, prepare the report and coordinate review without presenting a periodic report as an approval of future performance.

Frequently asked questions
Class I devices generally require a PMS Report under MDR Article 85, updated when necessary. PSUR requirements under Article 86 apply to Class IIa, IIb and III devices, subject to the applicable device framework.
Update it when necessary and at least every two years. Class IIb and III PSURs must be updated at least annually; urgent safety or vigilance actions follow their own timelines.
No. IVDR Class A and B devices use a PMS Report updated when necessary. IVDR Class C and D devices require a PSUR updated at least annually. Do not apply MDR class names to IVDs.
Where relevant, a report can cover an appropriate category or group, but document the grouping rationale and maintain traceable device-level data. A shared brand or sales category alone does not justify pooling conclusions.
